We've designed your application to be simple and ask only the questions required to estimate your coverage. You should be able to complete your application in less than fifteen minutes.

Once your application has been completed, you will be given the option to purchase your policy online with a credit card or by direct debit from your bank account. After payment is processed, you will receive your policy documents electronically by email.

We've made it easy for you to renew an individual insurance policy online. We've simplified your renewal application to ask only the questions required to estimate your coverage. And, unless you have significant changes to the type of work you do or to the coverage you need, you should be able to complete your renewal in less than fifteen minutes.

Once your renewal application is completed, you will be given the option to purchase your policy online with a credit card or by direct debit from your bank account. After payment is processed, you will receive your policy documents electronically by email.

At this time, a group policy (a policy that covers more than one person) cannot be renewed online. We will send you a paper renewal application approximately 90 days prior to your policy expiration date.

These changes will take place for insured members renewing October 1, 2013 and after. Most of the changes broaden the coverage provided, a few are restricting. Please review the changes and contact a Customer Service Representative if you have any questions on how coverage may be affected.

After you purchase a policy, you will receive an email containing a packet of information that includes: claim information, a proof of coverage certificate, and a complete insurance policy. If you have requested certificate holders or additional insureds in your online application, these will also be included.

Billing/charge information will show up on your financial statement as Lockton Insurance Online.

What was previously known as our “Virtual Service Center” will now simply be called “My Account,” and is where you can go to manage your account.

We’ll be sending your official renewal email from lockton_info@locktonaffinity.com when your policy is ready to renew. Please be sure to add lockton_info@locktonaffinity.com to your “safe senders” list.

If, for any reason, you are unable to complete the purchase or renewal process online, please contact us immediately to request a hard copy of the application. Please note that a $20.00 processing fee will automatically be applied to all paper applications submitted. Customer Service Representatives are available to assist you from 8:00 a.m. to 5:00 p.m. CST, Monday through Friday.

If your policy has lapsed less than 30 days:

At the first notice of a client injury, or potential claim, please contact your group’s Customer Service Representative, immediately. Our expert claim staff will advise you with the next steps in handling the situation, and will initiate the claims process if appropriate.

We know that being part of a claim can impact your practice and your peace of mind. Our dedicated claims team is here to walk you through the claims process, collect the necessary information, and get your claim started with the experts that will be working on your behalf.

Your policy provides claims-made coverage for your fitness professional services. You may report claims involving professional services you performed in previous years, as far back as this policy’s Retroactive Date (usually the date your uninterrupted claims-made coverage originally began), as long as there was no interruption in your coverage from that date forward.

The Limit of Liability on an insurance policy is the maximum dollar amount that the insurance company will pay if a claim is filed against you.

The Limit of Liability that you choose should correspond to the size and scope of your practice as well as the dollar amount you believe would cover a claim that could be made against you. You should include the expense of defense costs and settlements in your decision. Limits for this Professional Liability policy range from $250,000/$500,000 (limit per claim/annual aggregate limit) to $2,000,000/$4,000,000.

The limit per claim is the maximum dollar amount for damages and expenses that arise from claim. The annual aggregate limit is the maximum total dollar amount that your policy will pay in your defense, per year.

As your practice grows and changes—such as your level of education, certification or training expand, or you modify the services you offer—you should re-evaluate the limits of liability you carry. With claims-made coverage you can increase your coverage limits each policy year. Whichever limit you select at the start of each coverage period will apply to any covered claim that may be reported during the coverage period, regardless of the date of the incident.
